As a qualified bona fide member, you and your legal dependents can avail of consultations, be provided with medicines, treatment, hospitalization and examinations free-of-charge subject to availability of services and supplies at the Seamen’s Hospitals/ Clinics. Seamen’s Hospital does not handle medico-legal cases for members and their dependents.

However, it is prepared to give first aid treatment before the patient is transferred to a government hospital with a medico-legal officer or owners the procedure. You may visit our website at www.amosup. org for more detailed list of services offered by each branch.

Yes. If an active member/dependent request for a transfer of a patient to any AMOSUP Seamen’s Hospital, there are two basic procedures needed to be done:

  1. Verification of membership status of the seaman must be established. For telephone verification, always note the correct spelling of the vessel’s name to ensure proper verification. Always bring the required documents for verification purposes.
  2. Doctor-to Doctor coordination must be done prior to transfer to check for the availability of service, bed so as not to cause inconvenience to the patient.

No. The benefits is limited to the services units established under the FMDP program (Amosup Seamen’s Hospitals & Clinic) However, AMOSUP has introduced a Cost-Plus Arrangement under the Group Hospitalization Plan in partnership with Insular Life. The plan provides for the reimbursement of the actual, necessary, customary, reasonable and eligible expenses for covered disabilities for confinement including maternity-related cases like delivery, out-patient procedures, lens implant, radiation therapy and other medical conditions as de¬nied under this program. 

Basic considerations for coverage under the Cost-Plus Arrangement are (1) non-availability of service in the Seamen’s Hospital; (2) proximity to Seamen’s Hospitals; (3) emergency situation. The reimbursement has a maximum limit of P20,000 per family unit and may be availed only once a year. The policy on the eligibility of member applies. Meaning, only those with active membership are eligible for claim. 

All claims received with valid and complete documents will be processed and settled by Insular Life within seven (7) days 

Claims with lacking document will be communicated to Ms. Liza Panim or Ms. Anna Ibarra within three (3) days from the date of receipt of claims. 

Claim checks will be issued under the name of the seafarer.

INSTRUCTIONS FOR COST-PLUS ARRANGEMENT (UNDER THE GROUP HOSPITALIZATION PLAN) AVAILMENT:

  1. Application forms are available at the AMOSUP facilities indicated in  No.4
  2. Please fill up Parts I, III & IV of the form. Part I should be filled up by the member Part III by the Hospital (only if the Statement of Account from the Hospital is not detailed and wherein the room and board charges are billed separately from the other charges; Part IV by the Attending Doctor
  3. Please attach to the form the following: • Original official receipt (OR)  with TIN and original statement of account of the hospital/facility where payment for either confi¬nement or procedure/s is made.
  4. Submit the form together with the above required documents to any of the following facilities: AMOSUP Seamen’s Hospitals Business Office/Executive Office (Manila /Cebu/Iloilo/Davao/ Dagupan) 

AMOSUP Seamens Hospitals is composed of Level III and Level II Hospitals so availability of services may vary from one Seamens Hospital to another. Please refer to the chart for the availability of services.

Entitlement

The Seamen’s Hospitals seek to extend its quality service only to the true legal dependents of its members. To accomplish this, it needs to verify the legality of marriage contracts and birth certificates to guard against unauthorized use of the benefits.

All qualified members and their dependents can avail of benefits at the Seamen’s Hospital from the date of embarkation of the member up to six months after disembarkation. Please be reminded that it is only when you are onboard a vessel covered by an AMOSUP CBA that your ship owner is required to contribute to the funds that help run the Seamen’s Hospital and entitles you and your dependents to the benefits.

No. Vaccines are not included as free benefits in the Family Medical and Dental Plan. We only provide free first dose of BCG and Hepatitis B vaccines to newborns as part of the Hospital’s Newborn Package.

Requirements

Entitlement to the benefits under the Family Medical and Dental Plan (FMDP) is based on the membership status of the member. If you are an *active bona fide member, you or your qualified dependents can avail of the services of the hospital by going to the nearest AMOSUP Hospital branch and present for verification/screening purposes, the following documents upon registration at the Medical Records Section. Please note that each branch may differ/vary in their initial registration process such as the use of queuing system and the triaging of patients in the general outpatient department as practiced by Seamen’s Hospital Manila.

Seafarer’s legal dependents can apply for a Dependent’s Identification Card by submitting the following documents:

  1. Duly accomplished Dependent’s ID Application Form – issued by AMOSUP Seamen’s Hospitals  or  can be downloaded from the AMOSUP website(Family Data Registration Form)and
  2. Required documents listed under the dependent’s ID application- please link to separate file for Dependent’s ID
  3. Application is accepted, processed and released on the same day. However,  application with incomplete requirements will not be accepted and processed.

Instructions regarding the whereabouts of your assigned consultants will be provided by the resident physician or nurses on duty. Signages of Doctor’s Department and Offices are available throughout the Hospital. 

TELEHEALTH

The Telehealth service is a joint project of IMMAJ and AMOSUP, and the newest program made availableat the AMOSUP Seamen’s Hospital Manila.

This service is open to all seafarers, regardless of nationality, as long as they are on board a JSU/AMOSUP-IMMAJ CBA covered vessel and to all seafarers,regardless of nationality, on board Japanese-flagged vessels of IMMAJ members.

Members can avail telephone or email support if they are experiencing any medical or mental health concerns.

PEME

Once issued by the company with a written referral letter or has been given instructions to start their PEME with an understanding with the PEME Department. The seafarer may start his/her medical examination by:

Providing the PEME Staff with valid identification such as; AMOSUP ID, Seaman’s Book, passport, PRC ID and other Government issued ID’s with pictures for validation. 

The seafarer is then required to log their names at the DOH Master Logbook.

The Seafarer will fill out the complete PEME Information Slip for recording in the computer database indicating the nature of medical services to be provided. 

The PEME Card (Blue Card) is then provided to the seafarer to be presented to each of the hospital staff of the sections/ departments involved in the Pre-Employment Medical Examination.

All hospital sections/ departments shall ensure prompt results of PEME within seventy two (72) hours from time of examination.

The Examining Physician is responsible for evaluating the results and may require additional test/s, treatments, and or referral to Specialist for clearances prior to declaration of Final Recommendation (Fit or Unfit).Results of the PEME may not be within the normal or acceptable levels thus a referral to a specialist may be necessary.

Please note that the Specialist must indicate that you are cleared by affixing his notes and signature at the back of the PEME Card.

No. Pre-Employment Medical Examination will start once a written referral letter from the requesting company has been submitted or there is an initial verbal advice or email from the company stating that a written referral letter will be forwarded soon.

The results of the Pre-employment medical examinations shall be directly given to the client company provided that the confidentiality of patient’s information is observed and the seafarer has the right to secure a personal copy of his Medical Certificate for Service at Sea, Medical Examination Report for Seafarers and obtain records of treatment and other relevant medical information/ documents.

A fit to work PEME Certificate shall be valid only for ninety (90) days from the date of fitness or less as may be required by the host country/ employer prior to deployment.  If for any reason the applicant is not deployed within ninety (90) days after the date of fitness, the applicant shall be required to undergo a complete basic DOH PEME.

For seafarers, a fit for duty PEME Certificate shall be valid for maximum period of two (2) years, on the condition that the applicant is deployed within ninety (90) days from the date he or she was declared fit.  

 

Exception applies in the following cases : 1. Examinee is below 18 years old in which case the maximum period of validity shall be one (1) year: 2. Examinee has limitations due to health reasons.

LABORATORY

 

LABORATORY – (CBC, Urinalysis, Lipid Profile, HBA1C, Liver Profile, Cultures etc): You will be advised by your AMOSUP Physician or OPD Nurse on duty if your laboratory test will require you to go on fasting. A “Fasting Guideline Slip” will be given to you to learn and follow.

A. For laboratory test with fasting:

  1. While observing the fasting instructions provided by the Physician or OPD Nurse, please secure a queuing number from the security personnel at the main gate.
  2. After securing your queuing number, please proceed to the Waiting Area of the Extraction Room located at the Ground Floor of the South wing Building OPD Section.
  3. Wait for your number to be called before entering the Extraction Room. 
  4. Instructions for claiming of your test results will be provided to you by the receptionist from the extraction room. 

B. For laboratory test without fasting:

  1. Secure your queuing number from the security personnel at the main gate then proceed to the Waiting Area of the Extraction Room located at the Ground Floor of the South wing Building OPD Section.
  2. Wait for your number to be called before entering the Extraction Room. 
  3. Instructions for claiming of your test results will be provided to you by the receptionist from the extraction room. 

 

RADIOLOGY

 

I. Diagnostic X-Ray, Ultrasound, Mammography

  1. Once your request is generated in the Doctor’s Module by your in-house Attending Physician, please proceed to the Radiology Department Receptionist and have your procedure scheduled by presenting your Member’s ID / Dependent’s ID.
  2. Once scheduled, the Radiologic Technologist will perform the requested procedure as ordered by the Physician unless special preparations must be followed prior to the procedure.
    Note: Ultrasound examinations are scheduled procedures and information on preparation will be provided by the Radiologic Technologist.  X-Rays without preparations are done on the same day of the request while those with preparations are scheduled. 
  3. X-Ray results are released the following day while ultrasound results are released on the same day of the procedure.

II. CT-SCAN – MRI Sections

  1. Once your request is generated in the Doctor’s Module by your in-house Consultant, please proceed to the CT-SCAN – MRI Section and present your Member’s ID / Dependent’s ID, Allotment Slip and Seaman’s Book for verification.
  2. A request form will be provided to you along with other forms to be filled out. Verbal and written instructions regarding your preparations for your procedure will also be provided along with your schedule for the procedure.
  3. On your scheduled date, the radiologic technologist may ask you to sign consent if the procedure will involve contrast media while undergoing the procedure/s.
  4. Instructions regarding the release date of the result will be provided to you by the radiologic technologist on duty.

 

CARDIO-PULMO

(ECG, Spirometry, Stress Test, 2D Echo)

 

 (ECG, Spirometry, Stress Test, 2D Echo) – 

  1. Once your request is generated in the Doctor’s Module by your in-house Attending Physician, please proceed to the Cardio-Pulmo Reception except for the Spirometry which is located at the 2nd floor of the annex building and have your  procedure scheduled by presenting your request form and Valid AMOSUP Member’s and Dependent’s ID.
    Note: Please be advised that some procedures may have longer waiting time for scheduling than other procedures.
  2. If the procedure is available on the same day of the request, the OPD Nurse / Respiratory Therapist shall perform the requested procedure as ordered, otherwise it shall be scheduled on the soonest date available.
  3. On your scheduled date of procedure, please proceed to the Cardio-Pulmo Department and wait for your name to be called and have the procedure done.
  4. Instructions for claiming of your test results will be provided to you by the Cardio-Pulmo Personnel on duty. 

 

EENT

 

  1. Once your request is generated in the Doctor’s Module by your EENT Specialist, please approach the Nurse on duty for your instructions regarding the doctor’s order/s.
  2. The EENT Specialist may require you to take medications, undergo minor or major surgeries, test or procedures, or refer you to other consultants for further work-ups.
    -If the EENT Specialist only requires you to take medications, just claim your medicines from the Pharmacy (Please See FAQ on claiming medicines).
    -For minor and major surgeries, the EENT Specialist may require you to obtain a cardio-pulmonary clearance(Please see FAQ on processing request with the Laboratory, Radiology, and Cardio-Pulmo Department)and in special cases, refer you to other specialist as well if other health concerns are  during your consultation.
    -EENT test and procedures are usually done within the same day depending on the number of patients and the availability of Specialist who will perform the test or procedure.
    -If the EENT Specialist will require you to visit other consultants for further work-ups, please wait for the Nurse on duty to instruct you on the date of your follow up with the referred Consultant.
  3.   Once your cardio-pulmonary clearance is ready and you have been cleared by other specialist if any, or your medication regimen has concluded or the test / procedure required has been performed. Please see your in-house EENT Specialist by securing a queueing number from the security personnel from the main gate and wait for your number to be called by the Medical Records personnel to be encoded in the system for that day.
  4.   After being encoded at the Medical Records section, please proceed to the EENT Department and inform the Nurse on duty of your follow-up consultation and wait outside of the EENT waiting area for your number to be called before entering the doctor’s room.

 

ESWL

 

  1. Once your request is generated in the Doctor’s Module by your in-house Consultant, please wait for the OPD Nurse to have you scheduled for the ESWL procedure. 
  2. After the OPD Nurse provides you with the schedule of the ESWL procedure, further instructions regarding your preparations for the procedure will be given. ( Cardio-pulmonary clearance etc.)
  3. On your scheduled date of procedure, please proceed to the Emergency Room for other important instructions and to fill up important forms.
  4. Once preparations in the ER is complete, the ER Nurse will assist you going to the Operating Room and endorse you to the Operating Room Nurse for the ESWL Procedure.
  5. After the procedure, the OR Nurse will give you instructions for your follow up X-Ray or Ultrasound (depending on doctor’s order)  in which the results will be provided to you after the procedure.(Please see FAQ on processing request with the Radiology Department)
  6. On your Follow-up checkup, please get a queuing number from the security personnel from the main gate and wait for your number to be called by the Medical Records personnel to be encoded in the system for that day, then proceed to the OPD consultation  to see your in-house consultant.

 

NUCLEAR MEDICINE

 

  1. Once your request is generated in the Doctor’s Module by your in-house Attending Physician, please proceed to the Nuclear Medicine Department and inform the NucMed  personnel regarding your procedure by presenting your Valid AMOSUP Member’s and Dependent’s ID to generate a request form.
  2. Instructions will be given to you by the NucMed personnel on duty regarding  preparations prior to your procedure, so be sure to follow them to avoid any cancellations or delay in the procedure.
  3. You will be instructed to provide a contact number where the department can reach you and provide you with your schedule of the procedure.
    Note: Please be advised that some procedures may have longer waiting time for scheduling than other procedures.
  4. On your scheduled date of procedure, please get a queuing number from the security personnel from the main gate and wait for your number to be called by the Medical Records personnel to be encoded in the system for that day.
    Note:Please ask the Medical Records Personnel to encode your name under Dr. Eddie Lim of Nuclear Medicine.
  5. After being encoded in the Medical Records, please proceed to the Nuclear Medicine Department and wait for your name to be called and have the procedure done.
  6. Instructions for claiming of your test results will be provided to you by the NucMed Personnel on duty. 

 

REHABILITATION

 

  1. After your referral from the OPD-FAMED / Consultation, please proceed to the Rehabilitation Department for scheduling of consultation with an in-house Rehab Doctor. 
  2. On your date of scheduled consultation, please get a queuing number from the security personnel from the main gate and wait for your number to be called by the Medical Records personnel to be encoded in the system for that day.
  3. After being encoded at the Records Section, please proceed to the Rehabilitation Department and inform the Physical Therapist of your scheduled consultation with the Rehab Doctor.
  4. Should there be any order for treatment / management from the Rehab Doctor, the Physical Therapist on duty shall provide you with your treatment / management schedule and other instructions before the start of your regimen.
  5. On every scheduled day of your treatment / management, please do not forget to secure a queueing number from the security personnel at the front gate and have your name encoded at the Medical Records before proceeding to the Rehabilitation Department for your scheduled treatment / management.
  6. Once your program has concluded, a consultation with your Rehab Doctor is required to evaluate your condition and if further treatment / management is still required.

Please follow these simple steps when claiming your medications from our Pharmacy Department.

  1. After your consultation with the in-house OPD Physician / Consultant, your medications will be encoded in the system indicating the name and dosage of the drug.
  2. After the consultation, please proceed to the Pharmacy and present the following mentioned requirements to claim your medications, (Seamen: AMOSUP ID and Seamans Book or Contract of Employment, Verification Slip from records section Dependents: Dependent’s ID, Latest Verified Allotment Slip (original copy) Verification Slip from records section) otherwise, if provided with the “Request for Refill Booklet Slip” please proceed to the Director’s Office to claim the Refill Booklet.
  3.  After claiming your Refill Booklet, please proceed to your Attending Physician and present your Refill Booklet.
  4. Once your Attending Physician is done writing down all your medications in the refill booklet, please proceed to the pharmacy to claim your prescribed medications with the above mentioned requirements. The Refill Booklet will facilitate faster transactions with the Pharmacy Department every time you visit for a refill of your prescribed medications, and it will keep track of your medication history.     Note: Medications dispensed by the Pharmacy Department are regulated by the Policies and Procedures set by AMOSUP Seamen’s Hospital Manila.
    REFILL BOOKLET POLICIES:
    1. ONLY ACTIVE members and their dependents may claim the Refill Booklets.
    2. Prescribed medications may only be claimed three times a month, that is every 10 days starting from the last date of acquisition of medicines.
    3. Change in prescription may only be done by an in-house physician / consultant after a consultation.
    4. Always bring your Refill Booklet and requirements with you every time you visit AMOSUP Seamen’s Hospital for Medication Refill. If the Refill Booklet is not available upon your visit, you will be required to undergo consultation before being issued your prescribed medications and the amount of medication to be dispensed will be based on the minimum number of medicines allowed for that day.

Please follow these simple steps when claiming your medications from our Pharmacy Department.

  1. After the consultation with the OPD Physician/Consultant, the medications will be written in the Doctor’s prescription indicating the name and dosage of the drug.
  2. After the consultation, inform OPD Nurse of availability of Refill Booklet. Upon confirmation of Physician you will be provided of such.
  3. The patient may now proceed to the Pharmacy Department for the dispensing of the medications upon presentation of their refill booklet.
    Note: In case there is unavailability of the prescribed medication, the pharmacist may provide a drug with the same effectiveness as the one prescribed based on a list of drugs/formulary.
  4. The Pharmacist will then note the number of tablets/capsules dispensed and the signature/date of which the medications were given in the booklet.
  5. The Pharmacist would be allowed to issue medications sufficient for only 10 days.
  6. Prescription refills will be issued only every 10 days. If the amount prescribed has not yet been consumed, the patient will not be issued the succeeding amounts prescribed.
  7. Once all the prescribed amount of medicine has been consumed, the patient is required to follow-up at the OPD and reassessed by the physician for continuity of care and prescription of appropriate medications. The Physician should make sure that the quantity of medication to be prescribed to the patient must be for a month consumption of maintenance medication.
  8. In cases where the patient has new symptoms to consult, he/she consult first with the OPD Physician and must be comprehensively assessed for new conditions. The patient will be provided a new prescription for the new condition if warranted. Please see Refill Booklet Policies

    Note: Ultrasound examinations are scheduled procedures and information on preparation will be provided by the Radiologic Technologist. X-Rays without preparations are done on the same day of the request while those with preparations are scheduled.
  3. For ultrasound examinations, your examination shall be performed on the scheduled date and time provided by the Radiology Department Receptionist. Pre-procedural preparations must be strictly followed otherwise you will be rescheduled on the nearest possible date. 
  4. X-Ray results are release based on the availability of the Radiologist which is usually the following day after the procedure. Ultrasound results are released on the same day of the procedure.

 

REHABILITATION

  1. After your referral from the OPD-FAMED / Consultation, please proceed to the Rehabilitation Department for scheduling of consultation with an in-house Rehab Doctors together with the referral form from OPD.
  2. On your date of scheduled consultation, please proceed to the Rehab Medicine Department on or before the schedule clinic hour of the Rehab Doctors (TTh 10-12nn).
  3. After being examined by the Rehab Doctor, inform the Physical Therapist for the schedule of your therapy session.
  4. Should there be any order for treatment / management from the Rehab Doctor, the Physical Therapist on duty shall provide you with your treatment / management schedule and other instructions before the start of your regimen.
  5. On every scheduled day of your treatment / management, you need to be early as we only cater patients by scheduled time. 
  6. Once your program has concluded, a consultation with your Rehab Doctor is required to evaluate your condition and if further treatment / management is still required.

Please follow these simple steps when claiming your medications from our Pharmacy Department.

  1. After your consultation with the in-house OPD Physician / Consultant, Prescription will be written and Rx will be given to Pharmacy.
  2. Present to the Pharmacy the following mentioned requirements to claim your medications, (Seamen: AMOSUP ID and Seaman’s Book or Contract of Employment, Verification Slip from records section Dependents: Dependent’s ID, Latest Verified Allotment Slip (original copy) Verification Slip from records section)
  3. For Non-maintenance Rx, a 3 day medication regimen will be provided for Davao City based members and dependents. In an instance, a member / dependent wants to avail the remaining medication balance. He/She will go back to step 1 and provide an “UPDATED RX-Prescription”. For non-Davao residents, a full course medication regimen will be provided.
  4. For ANTI-TB Medications, the  patient will be given a 1 month supply of medicine which is refillable 2-3 days before his/her supply is fully consumed.  
  5. Maintenance medicines will be written in the Refill Booklet which will be provided by the OPDnurse.
  6. After claiming your Refill Booklet, please proceed to your Attending Physician and present your Refill Booklet.
  7. Once your Attending Physician is done writing down all your medications in the refill booklet, please proceed to the pharmacy to claim your prescribed medications with the above mentioned requirements. The Refill Booklet will facilitate faster transactions with the Pharmacy Department every time you visit for a refill of your prescribed medications, and it will keep track of your medication history.     Note: Medications dispensed by the Pharmacy Department are regulated by the Policies and Procedures set by AMOSUP Seamen’s Hospital Manila.

    REFILL BOOKLET POLICIES:
  1. ONLY ACTIVE members and their dependents may claim the Refill Booklets.
  2. Prescribed medications may only be claimed three times a month, that is every 10 days starting from the last date of acquisition of medicines.
  3. Change in prescription may only be done by an in-house physician / consultant after a consultation.
  4. Always bring your Refill Booklet and requirements with you every time you visit AMOSUP Seamen’s Hospital for Medication Refill. If the Refill Booklet is not available upon your visit, you will be required to undergo consultation before being issued your prescribed medications and the amount of medication to be dispensed will be based on the minimum number of medicines allowed for that day.
  5. Schedule for dispensing medications using the Refill Booklet is from Monday to Friday 8:00AM to 4:30PM and Saturdays 8:00AM to 11:30AM only.
  6. For DEPARTING MEMBERS, a 1 month worth of maintenance medicines will be provided as long as you can provide/present us a photocopy of your full flight departure details.
  7. Only patients are allowed to claim their medications from the Pharmacy.

Entitlement to the benefits under the Family Medical and Dental Plan (FMDP) is based on the membership status of the member. If you are an *active bona fide member, you or your qualified dependents can avail of the services of the hospital by going to the nearest AMOSUP Hospital branch and present for verification/screening purposes, the following documents upon registration at the Medical Records Section. Please note that each branch may differ/vary in their initial registration process such as the use of queuing system and the triaging of patients in the general outpatient department as practiced by Seamen’s Hospital Manila.

The Reception Area shall issue the Verification Slip, and will ask you to log your name in the Out-Patient Department for queueing purposes.

Once issued with the Verification Slip, the Reception Area will ask you to log your name in the patient’s OPD Logbook for queueing purposes.

All active members and their dependents are qualified to avail the medical and dental services under the AMOSUP Family Medical and Dental Plan (FMDP).

An active member is one who is on board a vessel covered by an AMOSUP Collective Bargaining Agreement. Under the FMDP the member’s classification as “active” will last until six (6) months from the member disembarked the AMOSUP CBA covered vessel. Once the six (6) month period has lapsed, and the member has not yet boarded another AMOSUP CBA covered vessel, the member shall be classified as an inactive member.

Accommodation of patients is done on a “first-come, first served” basis except for emergency cases wherein the patient is directed to the Emergency Room for immediate management.

The Seamen’s Hospitals seek to extend its quality service only to the true legal dependents of its members. To accomplish this, it needs to verify the legality of marriage contracts and birth certificates to guard against unauthorized availment of the benefits

A verification slip and Seaman’s book are necessary to determine the embarkation and disembarkation dates of the member so entitlement to the benefits can be verified.

Under the FMDP the member’s classified as “active” can avail of the benefit for date of embarkation up to six (6) months from the member’s  disembarkation from a AMOSUP CBA covered vessel.  When you onboard a vessel covered by an AMOSUP CBA the CBA provides that the  shipowner contribute to the FMDP  that help run the Seamen’s Hospitals and entitles you and your dependents to the benefits.

Refill Booklets are small notebooks with custom designed pages which are utilized for the purpose of acquiring, monitoring, and controlling the distribution and flow of medications for AMOSUP Members/Dependents who are regularly taking maintenance medications or are required to complete a course of prescribed medications by their attending physicians.

For Manila, Davao Iloilo, Cebu:

No. Vaccines are not included as free benefits in the Family Medical and Dental Plan. We only provide free first dose of BCG and Hepatitis B vaccines to newborns as part of the Hospital’s Newborn Package.

No. The benefit is limited to the hospital/clinic services units established under the FMDP program (Amosup Seamen’s Hospitals & Clinic). However, AMOSUP has introduced a Cost-Plus Arrangement in partnership with Insular Life.

AMOSUP has introduced a Cost-Plus Arrangement under the Group Hospitalization Plan in partnership with Insular Life. The plan provides for the reimbursement of the actual, necessary, customary, reasonable and eligible expenses for covered disabilities for confinement including maternity-related cases like delivery, out-patient procedures, lens implant, radiation therapy and other medical conditions as defined under this program. Basic considerations for coverage under the Cost-Plus Arrangement are (1) non-availability of service in the Seamen’s Hospital; (2) proximity to Seamen’s Hospitals; (3) emergency situation.  The reimbursement has a maximum limit of P20,000 per family unit per year.

TELE-HEALTH SERVICE

A joint project of IMMAJ and AMOSUP, Tele-Health is the newest program made available at the AMOSUP Seamen’s Hospital Manila. This service is open to all seafarers, regardless of nationality, on board JSU/AMOSUP-IMMAJ CBA covered vessels and to all seafarers, regardless of nationality, on board Japanese-flagged vessels of IMMAJ members.
