General Questions

All Filipino seafarers who boarded a vessel enrolled under AMOSUP Collective Bargaining Agreement is bonafide member and must acquire an AMOSUP membership card. All Qualified members can acquire the membership I.D. up to six months after disembarkation.

AMOSUP identification card (ID) is used in any of AMOSUP establishment such as hospitals, Sailors’ Home, maritime library, Slop Chest, recreation hall, Seamen’s Center, Seamen’s Village, training center and Provident Fund

Active member can apply for your membership I.D. on the following location:

  • MANILA – AMOSUP Records Section, G/F SEN’IN NO IE (Seafarer House) Building – Monday to Friday, 8am to12pm / 1pm to 3pm
  • CEBU – Sailors Home Building / Provident Fund Office, Monday to Friday, 8am to 4pm (no noon break)
  • ILOILO – Seamen’s Hospital Records Section, Monday to Friday, 8am to 12pm / 1pm to 3pm
  • DAVAO – JSU Building, IT Department Office, Monday to Friday, 8am to 12pm / 1pm to 5pm, Saturday, 8am to 12pm

Verification is an approved clearance from AMOSUP Records Department to ensure the dependent or the member is a bona fide member of the Union and to inform hospitals medical records department of the total registered vessel of the individual member covered by the Union. Members and dependent are always reminded that the verification must be secured first with AMOSUP Records Department.

  1. Dependents who can not present their Allotment pay slip and Dependents ID to the hospital.
  2. Members who can not present their Seamen’s registration book (Seaman’s Book) and membership ID to the hospital
  3. Members who last disembarked more than six (6) months and need to extend their hospitalization benefits.

Members who board vessels with an active AMOSUP CBA are reminded to report to AMOSUP Records Department to update their records upon disembarkation.

For members who downloaded the AMOSUP membership forms via website. Please submit your completely filled out forms with attached photocopy of any of the following:

  • Seamen’s Registration Book first page with your name and the last page with vessels name you boarded and date of disembarkation
  • Certificate of employment from the manning agent

To Records Department AMOSUP Seamen’s Center Building Cabildo Corner Sta. Potenciana Sts. Intramuros Manila for encoding. Submission of form is from Monday to Friday 8:00 am to 5:00 pm and on Saturdays from 8:00 am to 12:00 pm.

  • Seaman’s Book (page w/ last vessel’s name and sign off date stamp)
  • Latest pay slip / allotment slip
  • Updated passbook from Provident Fund
  • Certificate of Sea Service from manning agent
  • Last contract of employment
  • Endorsement Letter from manning agent
