To avail of our services, the patient should provide the following:
For members:
- AMOSUP ID
- Seaman’s Book
For dependents:
- Dependent’s ID
- Latest allotment slip or latest contract
Verification of their membership will be handled by our nurses. Once verified, the patient’s data will be collected into the patient’s logbook and medical chart with their initial assessment (vital signs and chief complaints). The patients and their chart will then be forwarded to our clinic physician for primary care.
Note: We also offer teleconsultation which started during this pandemic for continuity of our services. To avail this service, the patient should book an appointment thru our messenger account, AMOSUP DAGUPAN TELEMEDICINE and provide the above-mentioned requirements.
