Family Medical & Dental Plan

Your good health, as well as the well-being of your family are important to us. We understand that to be able to work well, you and your family have to be physically healthy.

This is the reason why AMOSUP created the Family Medical and Dental Care Plan (FMDP). Through the Seamen’s Hospital in Manila, a Level 3 hospital with teaching and training facilities, plus three other hospitals in Cebu, Iloilo and Davao plus a satellite clinic in Dagupan, dispense free-of-charge out-patient consultations, in patient hospitalization, medical procedures/operations as well as medications, among others, to members and dependents.

AMOSUP is continually looking for ways to improve the capabilities of Seamen’s Hospitals to address more of your health concerns and at the same time carefully studying strategic locations for satellite facilities where it can offer its services for the greater benefit of its members.

Since a decade ago, a nonstop modernization program of the hospitals has been implemented-the facilities and equipment are being expanded and upgraded to meet the best standards of healthcare delivery and family-oriented leisure hubs.

Through the vision of Capt..Gregorio S. Oca, a small medical clinic was established at the port area to serve the growing healthcare needs of the nation’s seafaring community. And by persistence, hard work, dedication and love for our seafarers who are braving the perils of the sea for their love ones at home. The small clinic at the port area was moved inside the walled city of Intramuros to gain more space and improve its medical and dental services to serve the growing numbers of Union members and their dependents.

Since then, the small clinic at the port area, is now the Nation’s Premier Maritime Medical Institution for Internationally recognized Healthcare Services, and Research which we now know as AMOSUP Seamen’s Hospital.

AMOSUP launched its TeleHealth service for its members onboard in 2016. TeleHealth allowed members to consult with doctors and specialists at the AMOSUP Seamen’s Hospital for emergency medical issues as it offers assistance to the ship officer in the diagnosis and treatment of ill or injured crew members onboard the vessel or offshore facilities where shore-based care is not possible.

In time for the World Mental Health Day 2018, the Associated Marine Officers’ and Seamen’s Union of the Philippines (AMOSUP) on 11 October 2018 launched a round-the-clock hotline that will cater to AMOSUP members experiencing issues or problems affecting their mental health aboard.

 The 24-hour service will be based at the AMOSUP Seamen’s Hospital. It currently has its own facility with three (2) phone lines 241-9465 at 242-9463 and Internet. 

 “Responders” who are composed of nurses, psychometricians, psychology and behavioral science graduates man the 24-hour hotline. They have undergone trainings on common mental problems, consultation psychiatry and workshops on interviewing techniques.

AMOSUP has introduced a Cost-Plus Arrangement under the Group Hospitalization Plan in partnership with Insular Life. The plan provides for the reimbursement of the actual, necessary, customary, reasonable and eligible expenses for covered disabilities for confinement including maternity-related cases like delivery, out-patient procedures, lens implant, radiation therapy and other medical conditions as defined under this program. Basic considerations for coverage under the Cost-Plus Arrangement are (1) non-availability of service in the Seamen’s Hospital; (2) proximity to Seamen’s Hospitals; (3) emergency situation.  The reimbursement has a maximum limit of P20,000 per family unit per year

The AMOSUP policy on the FMDP eligibility of member applies. Meaning, only those with active membership are eligible to claim the benefit from Insular Life. 

The accomplishing and filing of the claims should be within ninety (90) days from the date of discharge of the patient. 

All claims received with valid and complete documents will be processed and settled by Insular Life within thirty to forty-five (30 to 45) days

Claims with lacking document will be communicated to AMOSUP based on Insular Life’s evaluation. The seafarer and/or his dependent will be notified in case there are documents that need to be complied and fulfilled with.  

AMOSUP shall be informed by Insular Life of the approved claims and disbursement of check may be done at any Insular Life branches nationwide to be designated by the seafarer-member and/or his authorized dependent/representative. The benefit check will be issued to the name of the seafarer-member for deposit to his account only.

In case the seafarer is not available to claim the check, his dependent may claim the check, provided he can present an authorization letter and/or special power of attorney and a valid government-issued identification card of the seafarer.

INSTRUCTIONS FOR INSULAR’ S MEDICAL REIMBURSEMENT:

  1. Application forms are available at the AMOSUP facilities indicated in No. 4
  2. Please fill up Parts I, III & IV of the form.
    Part I should be filled up by the member
    Part III by the Hospital (only if the Statement of Account from the Hospital is not detailed and wherein the room and board charges are billed separately from the other charges;
    Part IV by the Attending Doctor
  3. Please attach to the form the following:
    • Original official receipt (OR) with TIN and original statement of account of the hospital/facility where payment for either confinement or procedure/s is made.
  4. Submit the form together with the above required documents to any of the following facilities:
