Dependents ID

The Dependents I.D. is likewise necessary in transacting with different AMOSUP operating units.

  1. Seafarer’s legal dependents can apply for Dependent’s Identification Card at the following locations and schedule:
    • MANILA – AMOSUP Records Section, G/F SEN’IN NO IE (Seafarer House) Building – Monday to Friday, 8am to12pm / 1pm to 3pm
    • CEBU – Sailors Home Building / Provident Fund Office, Monday to Friday, 8am to 4pm (no noon break)
    • ILOILO – Seamen’s Hospital Records Section, Monday to Friday, 8am to 12pm / 1pm to 3pm 
    • DAVAO – JSU Building, IT Department Office, Monday to Friday, 8am to 12pm / 1pm to 5pm, Saturday, 8am to 12pm
  2. Fill-out the Dependent’s I.D. application form. This can be obtained at the AMOSUP offices mentioned above.
  3. Application is accepted, processed and released on the same day. Personal appearance is required for I.D. picture capturing.
  4. Application with incomplete requirements will not be accepted and processed.

Payment: For Dependents Card

  • Free for new application
  • Replacement for lost/stolen Card
  • Php50.00 for 1st issuance of lost Card
  • Php75.00 for 2nd issuance of lost Card
  • Php100.00 for 3rd issuance of lost Card

For SINGLE

  • PSA Registered Birth Certificate of Member
  • PSA Registered Certificate of No Marriage (CENOMAR) of member
  • PSA registered Marriage Contract of Parent
  • PSA Registered of Birth Certificate of seaman’s siblings, 17 years old & below
  • Latest Allotment Slip

 

For MARRIED

  • PSA Registered Marriage Contract of member
  • PSA Registered Birth Certificate of unmarried children 17 years old & below
  • Latest Allotment Slip

For SINGLE PARENT

  • PSA Registered of Birth Certificate of seaman’s siblings, 17 years old & below
  • Original Copy of Court’s Decision on Annulment of Marriage, if separated
  • Latest Allotment Slip

For WIDOW/WIDOWER

  • PSA Registered of Birth Certificate of seaman’s siblings, 17 years old & below
  • PSA Registered Death Certificate of deceased spouse
  • Latest Allotment Slip

 

*Please Submit photocopy of the above requirements and place in a long brown envelope. Label it with the following information: LAST NAME, GIVEN NAME & MIDDLE NAME written at the upper left corner, back part of the envelope. Indicate the member’s or the applicant’s contact number under the member’s name.

*BRING ORIGINAL COPY OF ALL THE REQUIREMENTS for verification purposes.

*Photo of the ID holder will be taken on site via webcam

*ALWAYS BRING YOUR AMOSUP DEPENDENT’S I.D. AND LATEST ALLOTMENT SLIP WHEN AVAILING OF THE HOSPITAL BENEFITS.

FREQUENTLY ASKED QUESTIONS

If you are married, the following are your qualified dependents:

  • Your legitimate wife
  • Your legitimate unmarried children below 18 years old

If you are single with no children, the following are your qualified dependents

  • Your natural parents
  • Your unmarried brothers and sisters below 18 years old

If you are single with a child/child your qualified dependent/s is/are:

    • Your unmarried children below 18 years old

This is subject for approval. In case your marriage has been declared null and void or has been annulled, you can either;

  1. Submit a letter of request to the AMOSUP President’s Office and wait for approval or 
  2. Visit the AMOSUP Legal Department. 

Please bring the following requirements: 

  1. Annulment Documents; 
  2. PSA Marriage Certificate with remarks that your marriage is null & void. 

However, if you are only “Legally Separated” from your spouse, you cannot put your parents as your qualified dependent.

Yes, they need to secure their own AMOSUP dependents card for use in availing the benefits of the hospital and other transactions with different AMOSUP operating units.

You can apply for your Dependents I.D. at the AMOSUP Records Section, G/F SEN’IN NO IE (Seafarer House) Building – Monday to Friday, 8am to12pm / 1pm to 3pm. You can also get your dependents card at the following AMOSUP Offices CEBU, ILOILO and DAVAO.

Yes, you can also apply your AMOSUP Dependents Card in Cebu, Iloilo and Davao. Please bring the same requirements. 

Yes, they can go to AMOSUP records Department  and request for Hospital Verification. (note: this is a temporary clearance for the hospital, next time they visited the hospital they need to secure their dependent I.D. and bring the latest allotment slip or pay slip if they want to avail the hospital benefits.

Only legitimate unmarried children below 18 years old are qualified as your dependents.

No, the benefits are limited to the service units established under the FMDP (Family Medical and dental Plan) program (Amosup Seamen’s Hospitals & Clinic). 

You can ONLY use your hospital benefit to these following AMOSUP Hospitals and Clinic:

A. MANILA– AMOSUP Seaman’s Hospital Manila 
B. CEBU – AMOSUP SEAMEN’S HOSPITAL CEBU Camino Vicinal St. Umapad Mandaue City Cebu
C. ILOILO AMOSUP SEAMEN’S HOSPITAL ILOILO Onate St. Mandurriao IloIlo City, IloIlo
D. DAVAO GIG OCA ROBLE SEAMEN’S HOSPITAL R. Castillo St. Brgy. Centro Agdao, Davao City
E. DAGUPAN AMOSUP CLINIC – located at DAGUPAN DOCTORS VILLAFLOR MEMORIAL HOSPITAL

Please bring the latest allotment slip or last employment contract of the seafarer and your Amosup Dependents card.
