VACCINATION CARD
Your Vaccination Card will be available online at
https://amosup.upnextware.com
– Click “SEARCH MY RECORD”,
– Fill in your Mobile Number (registered in your 1st dose) and First Name,
– Then “VALIDATE
– At the right side of Personal Information, click Vaccination card logo for Printing.
