Who are my qualified dependents?

How long can I avail of this benefit?
It is only while the member is on board a vessel covered by an AMOSUP CBA or within six (6) months thereafter that he or his family can avail the benefits under Family Medical and Dental Plan (FMDP).

My husband is an AMOSUP member so why should I bring a verification slip or allotment slip?
The above documents are used by the hospital to determine the embarkation and disembarkation date of the member to verify entitlement to the benefit.

Why are members like myself and my dependents entitled to medical and dental services only when I am active and for six months thereafter?
It is only while the member is on board a vessel covered by an AMOSUP CBA or within six (6) months thereafter that he or his family can avail the benefits under Family Medical and Dental Plan (FMDP).

If I am admitted, who are the qualified watcher/s?
Only one watcher, preferably a relative, and of legal age is allowed to stay with the patient.

Why do we have to submit marriage contracts and birth certificates with registry numbers?
The hospital wants to extend its quality service to the true legal dependents of its members. To do this, it needs to verify the legality of the marriage contracts and birth certificates to prevent the unauthorized use of the benefits.

If a member or his qualified dependent is admitted in another hospital, can he be transferred to AMOSUP Seamen’s Hospital?
Yes, provided that:
- You are a qualified member/ dependent. Supporting documents/requirements are needed for verification purposes.
- Coordination by the patient’s Attending Physician to the Seamen’s Hospital Physician is facilitated prior to the transfer.
- Subject to room and services availability.
