How can I get AMOSUP ID?
- Member must be active (within 6 months from the date of disembarkation from an AMOSUP member vessel) in order to apply for the AMOSUP identification card.
- Submit any of the following requirements below
- Seaman’s Book o Latest Pay Slip / Allotment Slip and a valid ID
- Updated passbook from Provident Fund and a valid ID.
- Certificate of Sea Service from the Manning Agency and a valid ID.
- Last Contract of Employment and a valid ID.
- For lost AMOSUP identification card, secure Affidavit of loss
- Completely fill up the Application / Updating Form.
- Picture will be taken via web cam at the AMOSUP Records Department.
- A fee of Php10.00 for new AMOSUP ID and Php50. 00 for re-issuance of lost AMOSUP identification card.
- Issuance of AMOSUP identification card is open Monday to Friday, 8:00am – 3:00pm.
- You will receive your AMOSUP ID the same day.

How can I get my records validated or verified?
For members and dependents who need to use their hospital benefits and has no dependents ID or inactive members, please go to AMOSUP Records Department Seamen’s Center Bldg. For verification. Please bring any of the following for easy reference
- Latest Contract of Employment Certification from Shipping Agency
- Latest allotment slip / pay slip
- Any valid ID’s of Dependent and Member

Downloadable Forms
For members who downloaded the AMOSUP membership forms via website. Please submit your completely filled out forms with attached photocopy of any of the following:
- Seamen’s Registration Book first page with your name and the last page with vessels name you boarded and date of disembarkation
- Certificate of employment from the manning agent
To Records Department AMOSUP Seamen’s Center Building Cabildo Corner Sta. Potenciana Sts. Intramuros Manila for encoding. Submission of form is from Monday to Friday 8:00 am to 5:00 pm and on Saturdays from 8:00 am to 12:00 pm.
