    • Requirements for Discretionary Assistance

      Same requirements as to what WMBP benefits was requested.

    • Requirement/Procedure

      Union members and/or dependents who may seek legal advice from the AMOSUP Legal Officers fill out Legal Assistance Request Form and attach the necessary documents related to their concerns, such as, but not limited to:

      1. POEA Contract of Employment
      2. Masters report (notice of dismissal/repatriation)
      3. Grievance procedure/machinery on board
      4. Seaman’s Book

       

      These files can be sent to the Legal Affairs Office at AMOSUP Manila Office or via email (legal@amosup.org)

    • Requirements

      1. AMOSUP I.D.
      2. Personal letter addressed to our President, Dr. Conrado F. Oca, requesting for the withdrawal of your contributions;
      3. Letter of endorsement from the company/ies where you earned the contributions;
      4. Updated Provident Fund Passbook
      5. Photocopy of the Residence Certificate for the current year from barangay/city hall

      In cases of disability, the additional requirements are:

      1. Medical Certificate stating the type of sickness and declaring the member as permanently disabled to work as a seaman.

      In cases of death, the additional requirements are:

      1. NSO Death Certificate of the member;
      2. NSO Marriage Contract with register number; and
      3. AMOSUP Dependent’s I.D
      4. Cedula of the dependent for the current year
    • Procedure in applying for the benefit

      1. Upon the release of claim in the Death Benefit Section (For CBAs of JSU, TCC-IMEC, TCCC-NONIBF, Norwegian Model Agreement or upon filing for benefits in the Fil-Nor GAIN General Assistance and Information Office, Manila (For CBA of NIS Agreement ), the widow/beneficiary will submit all the requirements at the Widow’s pension Fund Section, 4th Floor, Seamen’s Center Annex Building;
      2. After twenty-four (24) months following the valid and due payment of the death compensation under the CBA, the Secretariat will send a letter to the widow/beneficiary informing of the benefit and their preferred branch of the Trustee Bank;
      3. The widow/beneficiary will fill out and return the attached letter informing the Secretariat of the bank branch address and other information;
      4. The Secretariat will advise the widow/beneficiary when to open an account with the branch of the Trustee Bank;
      5. The branch will coordinate with the Trustee Bank for payment of the monthly pensions.
